Our general enquiry form collects your name, phone number, optional email address, enquiry type, message, and your consent to be contacted. We record the enquiry's creation time and an internal processing status.
Do not submit medical reports, diagnoses, passport or citizenship numbers, or other sensitive health records. This form is for general enquiries only.
Why we collect it
We use your enquiry to respond to your questions, discuss available services, and arrange follow-up relevant to your request. The form does not subscribe you to marketing messages.
Storage and access
Enquiries are stored in a Cloudflare D1 database. Authorised clinic personnel can review them through secured Cloudflare management tools. There is no public enquiry dashboard. Cloudflare processes data to host and protect this website. The clinic must confirm its data access, international processing, and retention arrangements before publishing.
Security checks and external services
Cloudflare Turnstile helps protect the form from spam. Security verification may process your IP address and browser information. The embedded Google Map connects to Google when loaded, which may receive technical information such as your IP address. Retention and deletion
